RegulationsTextes applicables (ICPE, SEVESO, directive ATEX…)
Team membersAnalysis participants (function + role)

Select methods

Select only the methods you want to use. Only corresponding pages will appear in the navigation menu.

L'option Safety report enables PHA with PIGC grids (probability and severity) compliant with French SEVESO classified-facility regulations.

4Model the facility

Modelling structures the facility into a three-level hierarchy: systems → components → functions. This structure is reused across analysis pages to link every scenario to specific equipment.

Modelling page
Modelling page—systems / components / functions tree

Hierarchy

LevelExampleRole in the analysis
SystemMain reactor, Utilities, H₂SO₄ StorageGroups related components; filters in PHA, FMEA, and HAZOP
ComponentPump P-101, Valve V-204, Heat exchanger E-301Equipment targeted by FMEA failure modes
FunctionTransfer, Regulate pressure, Measure flowFunction performed by the component

Add elements

  1. Click + Add a system —enter a name and short description
  2. Select the created system, then click + Composant to attach equipment
  3. For each component, add one or more functions
  4. Repeat for each subsystem in your facility
The model does not need to be exhaustive before analysis begins. Expand the hierarchy throughout the study.
Systems and components defined here automatically populate dropdowns in PHA, FMEA, and HAZOP pages, avoiding re-entry and ensuring consistent labels.

5PHA—Preliminary Hazard Analysis

Polyodos provides two PHA variants based on your facility's regulatory context:

VariantUseRating grids
Physical-Risk PHAGeneral facility-hazard analysisSeverity / Likelihood configurable in Metrics
Safety-report PHAICPE and SEVESO facilities—hazardous phenomenaFixed regulatory PIGC grids
Physical-Risk PHA
Physical PHA—scenario list with inherent and residual ratings
Safety-report PHA

Physical-Risk PHA columns

ColonneDescription
System / EquipmentSelection from the model
HazardHazard source (fire, explosion, fall, leak, etc.)
Hazardous eventDirect consequence of the hazard
CausesOrigins that may trigger the event
ConsequencesImpacts on people, environment, and property
Inherent S / LSeverity and likelihood without measures
Preventive measuresExisting or planned barriers
Residual S / LRating after measures—residual risk level
StatutAcceptable / Monitor / Unacceptable / Critical

Enter a scenario

  1. Click + Add a row at the bottom of the table
  2. Select the system andequipment involved
  3. Enter the hazard, hazardous event, causes, and consequences
  4. Assign inherent S and L ratings—the criticality cell is coloured automatically
  5. Enter preventive measures and residual ratings
Utilisez l'assistant IA to automatically generate an initial set of scenarios from a text description of your facility.

6FMEA—Failure Modes, Effects and Criticality Analysis

FMEA identifies failure modes for each component, evaluates criticality through theRPN (Risk Priority Number = Severity × Occurrence × Detectability), and defines corrective actions.

Page AMDEC
FMEA page—failure-mode table with initial and residual RPN

Colonnes AMDEC

ColonneDescription
System / Component / FunctionTaken from the facility model
Failure modeHow the component can fail (e.g. leak, blockage, drift)
EffetsConsequences for the system and people
CausesOrigins of the failure
G / O / D brutsSeverity, Occurrence, Detectability—initial RPN calculated automatically
Mesures correctivesPreventive actions, corrective actions, or improvements
Residual S / O / DRatings after measures—residual RPN
The RPN is calculated automatically as soon as all three values (S, O, D) are entered. Rows whose RPN exceeds the critical threshold are highlighted.

Filter and sort

Use the column-header filters to quickly isolate critical failure modes or focus on a particular system. Sorting by decreasing RPN identifies action priorities in one click.

7HAZOP — Hazard and Operability Study

HAZOP analyzes process-parameter deviations from nominal conditions by combining guide words (No, More, Less, Reverse…) with parameters (flow, pressure, temperature, level…).

Page HAZOP
HAZOP page—deviations by node, with causes, consequences, and recommendations

Colonnes HAZOP

ColonneDescription
NodeSection or equipment analyzed (e.g. P-101 pump supply line)
ParameterProcess variable concerned (flow, pressure, temperature…)
Guide wordType of deviation (No, More, Less, Other than…)
DeviationParameter + guide word combination (e.g. No flow)
CausesPossible origins of the deviation
ConsequencesEffects of the deviation if uncorrected
Mesures existantesExisting safeguards
G / VDeviation rating
RecommandationsAdditional actions to implement
Organize your HAZOP by nodes. A node is a homogeneous process section with stable operating conditions. Filter the node list in the table for quick navigation.

8LOPA — Layer Of Protection Analysis

LOPA quantifies the frequency of hazardous events by accounting for independent protection layers (IPLs) and checks whether the residual risk is tolerable.

Page LOPA
LOPA page—scenarios with initiating frequency, IPLs, and residual frequency

Colonnes LOPA

ColonneDescription
ScenarioDescription of the hazardous event analyzed
Initiating eventTriggering cause (e.g. power loss, operator error)
Initiating frequency (fi)Initiator occurrence frequency (events/year)
ERCTop event
Consequence / SeverityImpact and severity level of the top event
IPLIndependent protection layers with their PFD (Probability of Failure on Demand)
Residual frequencyfi × PFD(IPL1) × PFD(IPL2)…—calculated automatically

Add IPLs

  1. In a LOPA row, click + IPL to add a protection layer
  2. Select the type IPL type (BPCS, SIS, relief valve, dike, procedure…)
  3. Enter the PFD (e.g. 0.1 for a SIL 1 SIS)
  4. The residual frequency is recalculated automatically after every change
An IPL must be independent of the initiating event and the other IPLs. Do not count the same equipment multiple times under different names.

9Bow-Tie

The bow-tie graphically represents a top event, with causes on the left and consequences on the right, showing prevention and protection barriers.

Bow-Tie page
Bow-Tie page—advanced-mode diagram with logic gates and barriers

Two diagram modes

ModeCas d'usageFeatures
Simple Communication, executive presentation Visual bow-tie envelope, cause/consequence barriers, without probabilities
Advanced Analyse quantitative, lien LOPA AND/OR gates, conditions, probabilities, minimal cut sets (MOCUS), PHA/safety-report import

Create a diagram

  1. In the side panel, click + Simple or + Advanced as required
  2. Name the diagram and define thetop event in the center
  3. Add the causes on the left (right-click the top-event node → Add a cause)
  4. Add the consequences on the right
  5. Place barriers on the links (click a link → Add a barrier)
In advanced mode, use Importer depuis APR/EDD to automatically retrieve causes and consequences already entered in your PHA tables.
Each diagram has a fixed mode set at creation. To change modes, create a new diagram—the two can coexist on the same page.

10Metrics and risk matrix

The Metrics defines the rating scales (Severity, Likelihood) and criticality matrix used throughout the project analyses.

Metrics page
Metrics page—configurable scales and criticality matrix

Configurable elements

ElementDescription
Severity levelsNumber of levels (3 to 5), labels, and colors
Likelihood levelsNumber of levels, labels, and reference frequencies
Criticality matrixColor of each S×L cell (Acceptable / Monitor / Unacceptable / Critical)
Risk levelsLabels and FMEA RPN thresholds
Changing metrics affects every existing rating in the project. Define the scales before entering scenarios.
En mode Safety Report (SEVESO), the regulatory PIGC scales are used for safety-report PHA—they are fixed and separate from configurable metrics.

11Generate the report
