4-FMEA
The Failure Mode, Effects and Criticality Analysis (FMEA) analyses components one by one according to the IEC 60812 standard.

FMEA type
Choose the type at the top of the page:
- Functional — analysis by service function (System → Service function)
- Component — analysis by equipment (System → Component → optional Functionality)
- Process — analysis by operating phase (System → Process phase)
Column selector (☰ Columns)
| Toggle | Columns affected |
|---|---|
| Detection means | "Existing detection means" and "Detection means to add" columns |
| Actions to consider | S'/O'/D'/RPN' residual rating columns and additional preventive actions |
| Corrective actions | Column for already identified corrective actions |
| Notes | Free notes column |
| Grid | Cell borders |
TIP
Enabling Actions to consider automatically enables the S'/O'/D'/RPN' residual rating — it's the way to track how effective the action plans are.
Functionality toggle (Component type only)
In Component mode, an additional toggle appears below the toolbar: Functionality — displays a detail column for the function performed by the component in the analysed scenario.
FMEA table columns
| Column | Description |
|---|---|
| System | Selected from the modelling |
| Component / Function / Phase | Depending on the selected FMEA type |
| Failure mode | How the element can fail its function (e.g. Stuck in closed position) |
| Effects | Consequences of the failure on the system or process |
| Causes | Root causes of the failure (wear, corrosion, adjustment error…) |
| S | Severity of the effects (1 to N) |
| O | Occurrence — probability of the cause occurring (1 to N) |
| D | Detection — ability to detect the failure before an incident (1 to N, reversed) |
| RPN | Risk Priority Number = S × O × D (calculated automatically, colour-coded) |
| Corrective actions | Actions decided to reduce the RPN |
| S' / O' / D' / RPN' | Residual rating after actions (if "Actions to consider" is enabled) |
✦ AI button on corrective actions
An inline ✦ AI button appears on the Corrective actions column (if AI is enabled). It generates targeted corrective action suggestions for that specific failure mode.
High RPN
A high RPN flags a priority component. Aim to bring critical RPNs below the threshold configured in FMEA Metrics. The action can target S, O or D, whichever is most technically realistic.
FMEA Metrics tab
Configure the S, O, D scales and the RPN thresholds (criticality levels with min–max ranges and colours).
Risk matrices tab
S × O matrix, with failure modes positioned on it. Raw view and residual view.
Corrective actions tab
Consolidated table of all actions arising from the FMEA, with owner, due date and status.
Guide tab
Built-in contextual help: recommended scales, failure mode examples, process FMEA vs production equipment FMEA criteria.
