2-PHA — Physical risks
The Preliminary Hazard Analysis (PHA) for physical risks identifies accident scenarios linked to common hazards (fire, explosion, fall, …).
Toolbar
| Button / Control | Function |
|---|---|
| ☰ Columns | Opens the visible columns selector (see below) |
| 🏭 Modelling | Generates PHA rows for a selected system, by crossing its components with the hazards from the library. To be refined manually |
| 📋 Excel | Pastes a table copied from Excel — recognises columns by position |
| ⬇ XLS | Exports the PHA table to an Excel file (.xlsx) |
| 🎨 | Opens the rating column colour settings |
| 👁 View / ✏ Edit | Toggles between read mode (frozen cells, more readable) and edit mode |
| ✦ AI | Opens the AI generation panel (only visible if AI is enabled on the project) |
Column selector (☰ Columns)
Toggles to show or hide optional columns:
| Toggle | Column affected |
|---|---|
| Hazard | "Hazard" column (hazard type: thermal, mechanical, chemical…) |
| Dangerous situation | Intermediate column between the cause and the feared event |
| Actions to plan | G', P', C' columns (residual rating) and Status — enable to track actions |
| Status | Status column alone (Acceptable / To monitor / Not acceptable / Critical) |
| Notes | Free notes column per row |
| Grid | Displays the table's cell borders |
PHA table columns
| Column | Description |
|---|---|
| # | Sequential number |
| System / Equipment | Selected from the modelling (drop-down list) or entered freely |
| Hazard | Hazard type (thermal, mechanical, chemical, electrical, pressure…) — from library list |
| Causes | Root causes of the event (equipment failure, human error, external cause) |
| Feared event (FE) | The potential accident (e.g. Pool fire, UVCE explosion) |
| Consequences | Effects on people, the environment, assets |
| Existing measures | Prevention and protection barriers in place |
| G | Raw severity (1 to N according to the configured metrics) |
| P | Raw probability |
| C | Raw criticality = G × P (calculated, colour-coded according to the matrix) |
| Actions to plan | (if option enabled) Additional reduction actions |
| G' | Residual severity after actions to plan |
| P' | Residual probability |
| C' | Residual criticality |
| Status | Acceptable / To monitor / Not acceptable / Critical |
✦ AI panel (generation from modelling)
Click ✦ AI in the toolbar to open the generation panel:
- Engine — select Gemini, Claude or another configured engine (see AI Configuration)
- API key — enter your key (stored locally, not needed with Ollama)
- System selection — select the system to analyse among those in the modelling
- Click Generate — the AI proposes PHA scenarios for that system
- The proposals are displayed as a checklist — uncheck any that aren't relevant
- Click Import (N) to add the selected scenarios to the table
PHA Metrics tab
Configure the G and P scales and the criticality matrix specific to the PHA (independent of the FMEA or HAZOP metrics if you want to differentiate them).
Risk matrices tab
Severity × Probability matrix with all scenarios positioned on it. Raw / Residual toggle to compare before and after actions.
Actions tab
Consolidated table of all actions entered in the PHA, with status and owner tracking.
